Anchor Care, Inc. maintains anonymous reporting channels for its employees, its contractors, and staff of any vendor or downstream entity working on Anchor programs. Reports may be made without giving a name.
For fraud, waste, or abuse, privacy or security incidents, quality of care or member safety, falsified records, conflicts of interest, retaliation, and any other compliance concern. Reviewed by Anchor's Compliance Officer.
Open the reporting formFor any concern that involves Anchor's Chief Executive Officer. Reviewed only by the Board designee, an advisor who is not part of Anchor's management.
Open the CEO concerns formNeither form collects an email address, requires a Google sign-in, records a Google account, or accepts file attachments. A reporter may give a name or leave it out.
Two things are outside Anchor's control. The first is the reporter's device and network; for the strongest protection, submit from a personal device on a non-work network. The second is inference: Anchor is a small team, and details in a report can point back to the person who made it.
This page does not use analytics, advertising, or tracking tools, and the form links open without passing on which page they came from.
Anchor prohibits retaliation against anyone who reports a concern in good faith or takes part in an investigation. This applies to employees, contractors, and vendor staff. Retaliation is itself a reportable violation.
The Compliance Officer acknowledges and triages each report within two business days. Reports concerning the Chief Executive Officer go only to the Board designee, who follows the same time frames and may engage outside counsel at Anchor's expense. Where a report involves a health plan's members or contract, Anchor reports it to that plan within the time frame the agreement requires. Every report, and every action taken on it, is recorded in an access-controlled log retained for ten years.
A report that concerns the Compliance Officer may be sent instead to Brenda Schmidt, Founder and CEO, at brenda@anchorcare.ai.
The main reporting form invites the reporter to create a passphrase. Anchor posts the status of each report against its passphrase on a view-only status page, which shows a passphrase and a status only. Anchor cannot recover a lost passphrase. A status of "More information needed" asks the reporter to file a second report using the same passphrase. Reports about the Chief Executive Officer do not appear on the status page; a reporter who wants a reply from the Board designee may leave contact information in that form.
No one is required to report to Anchor first. Concerns may be reported directly to the health plan's compliance hotline, to the Centers for Medicare & Medicaid Services, or to the HHS Office of Inspector General at 1-800-HHS-TIPS (1-800-447-8477) or oig.hhs.gov. Reporting outside Anchor is protected from retaliation in the same way.
These channels operate under Anchor's Compliance Program procedure CP-003, Appendix A.
